Use the notification forms below for the following sexually transmitted infections:
- Acquired Immunodeficiency Syndrome (AIDS) - Use this form to notify
- HIV (Human Immunodeficiency Virus) - The laboratory will notify the case, and the ordering health practitioner will be sent a questionnaire to complete and return.
- Gonorrhoea - Use this form to notify
- Infectious syphilis - Use this form to notify
- Congenital syphilis - Use this form to notify
